On June 27, 2019, the field meeting of the Council under the Government of the Russian Federation on the issues of trusteeship in the social sphere on the topic: “On the development of a palliative care system, quality improvement he and accessibility of analgesic therapy” was held at the main production site of the FSUE Moscow Endocrine Plant.
The meeting, which discussed the organization of palliative care, was attended by the Prime Minister Dmitry Medvedev. Before the meeting, Dmitry Medvedev and Tatyana Golikova examined the workshops for the production of invasive drugs and transdermal therapeutic systems. The management of the Government of the Russian Federation saw how in modern high-tech pharmaceutical production drugs are filled into vials and the vials are automatically monitored for microcracks, mechanical inclusions and filling volume.
Dmitry Medvedev noted at the meeting that he also examined the workshop for the production of transdermal therapeutic systems, where the release of one of the most effective Russian analgesics is organized. "This is a real import substitution with all the resulting points," emphasized the Chairman of the Government of the Russian Federation.
He asked Mikhail Fonarev, the Director General of the FSUE Moscow Endocrine Plant, to tell the meeting participants what had been done and would be done in the production of anaesthetics. Dmitry Medvedev approved the development plans of the FSUE Moscow Endocrine Plant for the production of a full cycle of drugs and noted the need to achieve 100% provision of palliative patients with anaesthetics.
The Chairman of the Government of the Russian Federation expressed confidence that after consideration of the issues identified during the meeting, proposals would be prepared that would become the basis for the preparation of regulatory legal acts and emphasized that all these proposals would be worked out by the Government of the Russian Federation.
“I think that today you will discuss the most diverse issues that arise here, including issues of legislation improvement. Moreover, both the legislation connected with the use of appropriate dosage forms, and some other laws, as I understand it, including criminal law,” Dmitry Medvedev specified.
During the meeting Tatyana Golikova noted the importance of drug support for citizens who need anesthesia, including expanding of the range of appropriate drugs produced in Russia. “We will need to resolve the issue of drug support in full. The fact is that Russian production will move further towards import substitution, and we will have complete dosage forms that are necessary to provide palliative care to adults and children population by 2023”.
Tatyana Golikova also touched the topic of decriminalization of the responsibility of health workers for violations of the circulation of narcotic and psychotropic substances. She suggested to discuss a mechanism in which the affairs of doctors connected with the accidental loss or destruction of the medicines containing drugs would be considered by special commissions created within medical organizations.
The key problem, according to all participants of the meeting, is the problem of assistance to palliative patients at home. Field palliative services in most regions are poorly developed. In addition, there is a problem with the prescription of anaesthetics outside the hospital, as well as with the organization of respiratory support.
The Deputy Prime Minister of the Russian Federation also said that the Russian Ministry of Health is currently working on a procedure that allows filed medical teams to have signed prescription forms. In combination with measures that prevent abuse this should solve the problem of prescription of narcotic and psychotropic drugs to patients who need them for pain relief at home.
In the concluding remarks the Deputy Prime Minister of the Russian Federation Tatyana Golikova noted that a lot has been done during the reporting period, but more needs to be done to provide people who need it with high-quality palliative care.
