The problem of increasing the availability and quality of pain relief has become particularly acute in the last decade In the Russian Federation, largely due to the dynamic development of palliative medicine.
Provision of medicines for patients in need of pain relief, due to its high social significance, is within the field of special attention of state authorities and civil society.
Starting from 2018, the Government of the Russian Federation annually allocates additional federal funding to organize palliative care in all constituent entities of the country. These funds are directed, among other things, to the purchase of modern dosage forms of opioid analgesics.
In 2020, a new Action Plan (“Roadmap”) “Improving the quality and accessibility of palliative care” until 2024 aimed at improving the quality of life of patients in need of palliative care, increasing patient satisfaction with the availability of medications, including those containing narcotic drugs and psychotropic substances (NDPS), was approved.
In pursuance of the “Roadmap”, FSUE “Endopharm” annually expands nomenclature of domestic narcotic and psychotropic drugs. Since 2016, the Company has registered 12 new medicines used for delivery of palliative care, according to 8 international non-proprietary names (INN) in 10 dosage forms. FSUE “Endopharm” plans to introduce 6 new medicines used for delivery of palliative care until 2024. Thus, medical professionals will access to 36 domestic narcotic and psychotropic drugs in 12 dosage forms, including those used in children.
From September to November 2022, the second stage of an educational project for specialists in palliative care organized by the Association of Professional Participants in Hospice Care and the Sechenov First Moscow State Medical University of the Ministry of Health of Russia together with FSUE “Endopharm” is being held. Scientific and practical seminars dealing with treatment of chronic pain syndrome in adults, including with the use of opioid analgesics, are held in 10 constituent entities of the Russian Federation. Seminar participants obtain up-to-date knowledge on the modern classification and diagnosis of types of chronic pain syndrome, socio-psychological aspects of pain treatment, the chronic pain treatment syndrome principles and algorithms, and the range of medicines available for palliative care. The issues of legal regulation of administering and issuing NDSP are of particular regard. Practical master classes on the drawing up prescription forms are held as part of the seminars.
In September 2022, with the assistance of FSUE “Endopharm”, an educational medical forum within the framework of the project “Development of expertise of specialists in palliative care” was held. Leading experts shared relevant and useful information about the main results of the development of the palliative care system in the Far Eastern Federal District. The speech of Yuri Sukhanov, the Chief Freelance Specialist in palliative care of children of the Moscow Department of Health on the topic “Transmucosal medicinal benzodiazepines: a doctor’s experience and parents’ impressions” arosed the greatest interest of the audience.
In October 2022, in Vladimir and Tver, with the support of FSUE “Endopharm”, trainings on the topic “Issues of medicine provision for children in need of palliative care with transmucosal psychotropic drugs” were held.
The seminars touched upon topical aspects of organizing the provision of specialized medical care to children with neurological pathology. In particular, the topics of reports were new opportunities for the use of non-invasive forms of benzodiazepines in the provision of palliative care to children, transmucosal forms of benzodiazepines – a modern approach to the treatment of seizures, status epilepticus in children: the role of benzodiazepines in solutions for rectal and oromucosal use in arresting and prevention. In August 2022, these medicines were included in the updated clinical guidelines KR741_1 “Epilepsy and status epilepticus in adults and children”.
A set of measures taken for 9 months of 2022, compared to the same period last year, resulted in a 25% increase in disbursement of narcotic drugs in invasive dosage forms by the constituent entities of the Russian Federation, and in a 7% increase – in non-invasive dosage forms of prolonged action.
